What we do

HSWs provide person-centered home support services that promote optimal functioning in the skills and activities of daily living and enhance the quality of life of individuals.
HSWs work as part of an interdisciplinary team that enables Yukoners to remain in their own homes as long as possible while receiving evidence-based quality health and personal care.

Job description

•    Provides personal care and assistance with activities of daily living such as bathing, dressing, toileting, shopping, meal preparation, foot care and facilitating community access and socialization;
•    maintains awareness of clients’ abilities, diet, diagnosis, personal leisure interests and special needs by reading the health care records and discussing issues with the client care coordinator or supervisor prior to implementing treatment;
•    performs client-specific tasks under the direction and approval of a therapist or nurse, such as exercise programs or wound care;
•    completes approved tasks as outlined in Medication Assistance Program policy and performs client-specific advanced skills based on applicable policies and training, such as bowel routine;
•    documents in the client health record, communicates changes in client status to the care team and makes recommendations for changes to the client care plans;
•    performs regular safety monitoring, basic maintenance and cleaning of clients’ homes and treatment equipment. Reports concerns to client care coordinator;
•    provides respite to the primary caregiver by caring for the client while the caregiver is out of the home;
•    provides light housekeeping duties within the care plan to maintain a safe and sanitary environment, such as cleaning bathrooms, vacuuming and washing floors; and
•    provides personal care at end of life for clients in their homes. This includes providing comfort measures to the client and providing emotional support to their family members.

Job requirements

•    Certificate as a personal support worker (PSW), home support worker (HSW) or nursing home attendant (NHA) from a recognized Canadian educational institute. Equivalent experience may be considered;
•    recent experience working in home care or a community-based setting;
•    recent experience providing personal care, home management and respite care in a community setting;
•    TB screening;
•    vulnerable sector RCMP security clearance;
•    CPR level C (course taken within the last 2.5 years at time of hire);
•    food safe level 1 (completed within the last 2 years); 
•    WHMIS 2015; and
•    valid Yukon driver’s license.
